Schedule an Appointment

Appointments are available for virtual nutrition consultations or in-home lactation consultations. In order to schedule an appointment, please complete the following questions below. We are in-network with Blue Cross Blue Shield of Texas, United Healthcare and partner with a billing entity to expand coverages. Please be sure to include your insurance information if you would like to verify coverage for services. Verification of insurance is not a guarantee of payment.

NUTRITION: We are in-network with Blue Cross Blue Shield of Texas and United Healthcare. Coverage varies for each plan. The patient is responsible for out-of-pocket costs required by their insurance plan (co-pays, co-insurance, etc.). Please provide your insurance information in the Inquiry for Support form for verification of benefits.

LACTATION: We are in-network with Blue Cross Blue Shield of Texas and United Healthcare. We also partner with a billing entity to accept various plans. Coverage varies for each plan. The patient is responsible for out-of-pocket costs required by their insurance plan (co-pays, co-insurance, etc.). Please provide your insurance information in the Inquiry for Support form for verification of benefits.

Verification of benefits is not a guarantee of payment. It is recommended that you also call to verify nutrition services are covered: Verifying Insurance Coverage. Some plans may require a referral from the patients Primary Care Physician for services to be covered by insurance. We can assist with the referral request as needed.

OUT-OF-NETWORK

Cash pay and HSA/FSA payment options are available for services not in-network. Payment in full is required before the appointment time. Customized packages are available upon request.

Nutrition

Initial Consultation (60-75 min): $165

Follow-up Consultation (30-45 min): $80

Lactation

Initial Consultation (90 min): $198

Follow-up Consultation (60 min): $150

Prenatal Consultation (60 min): $150

We are happy to accommodate in-home consultations based on availability in our servicing area. Travel/Convenience fees vary based on location.
